Job summary

Premier Washington Rehabilitation & Nursing Center in Washington, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation department. You will evaluate residents, create personalized treatment plans, and collaborate with a multidisciplinary team to improve patient outcomes.

Qualifications

• PA licensed Physical Therapist required.
• Graduate of an accredited Physical Therapy program.
• SNF/long-term care experience preferred; new grads welcome.
• Experience with Net Health EMR is a plus.
• Strong communication, organization, and time management skills.
• Passion for patient-centered care.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy to skilled nursing and long-term care residents.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members to optimize patient outcomes.
• Document evaluations, treatment sessions, progress notes, and discharge summaries using Net Health EMR.
• Participate in care conferences and communicate patient progress with families and team members.
• Ensure compliance with state, federal, and company regulations.

Premier Washington Rehabilitation & Nursing Center is seeking a compassionate, motivated, and dedicated Full-Time Physical Therapist (PT) to join our growing in-house rehabilitation department. If you're passionate about helping patients regain independence and improve their quality of life while working alongside a collaborative interdisciplinary team, we'd love to hear from you!

What You'll Do

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy services to skilled nursing and long-term care residents.
