Job summary

Bradford Hills Nursing & Rehabilitation Center is seeking a dedicated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Troy, PA. This SNF/LTC setting emphasizes patient-centered care with an interdisciplinary team and consistent caseload.

The PT will evaluate residents, develop customized plans, and collaborate with physicians and nursing staff to improve mobility and independence while ensuring compliant documentation.

Qualifications

• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong clinical, communication, and documentation skills.
• Commitment to patient-centered care.
• SNF/LTC experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence.
