Job summary

Watsontown Nursing and Rehab Center is seeking a dedicated Physical Therapist (PT) for a full-time position in Watsontown, PA. The in-house rehabilitation team delivers person-centered care, developing customized treatment programs to maximize residents’ independence and outcomes.

The PT will evaluate, plan, and implement evidence-based interventions, collaborate with the interdisciplinary team, and ensure timely documentation in accordance with state and federal regulations.

Qualifications

• SNF or LTC experience preferred.
• Current physical therapy license in PA.
• Strong clinical and interpersonal skills.
• Ability to collaborate with IDT and families.

Responsibilities

• Consult patients to understand rehab goals and needs.
• Perform evaluations and interpret results.
• Develop individualized treatment plans and progress notes.
• Administer PT treatments per plan of care.
• Monitor responses and adjust treatment with IDT.
• Document accurately per policies and regulations.
• Attend required rehab department meetings.

Watsontown Nursing and Rehab Center has an exciting opportunity for a Full-timePhysical Therapist (PT) in Watsontown, PA!

We are looking for a dedicated PT to join our in-house rehabilitation team of outstanding professionals who provide person-centered care to our residents and patients. Our goal is to help each individual achieve meaningful outcomes while maintaining the highest standards of quality and compassion.

Our staff works together to develop customized treatment programs and best practices designed to meet the unique needs of each resident and exceed industry standards.

Physical Therapist Responsibilities

• Consult patients to understand their individual needs, concerns, and rehabilitation goals.
• Perform comprehensive patient evaluations and assess and interpret evaluation findings and test results.
• Develop individualized treatment plans using a variety of evidence-based physical therapy techniques and interventions.
• Create personalized rehabilitation programs designed to help patients achieve their functional goals and maximize independence.
• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, increase mobility, and promote healing.
• Ensure all treatment is provided in accordance with the established plan of care.
• Monitor patient responses to treatment interventions and modify treatment approaches as clinically appropriate in collaboration with the interdisciplinary team (IDT).
• Provide clinical support, education, and guidance to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to promote coordinated, high-quality patient care.
• Ensure all documentation is completed accurately, thoroughly, and in a timely manner in accordance with company policies and applicable state and federal regulations.
• Comply with all applicable rules, regulations, policies, procedures, and professional standards.
• Attend required meetings and participate in other duties as designated by the Director of Rehabilitation.

Physical Therapist Skills & Qualifications

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.
• Current knowledge of physical therapy treatment practices, techniques, and interventions.
• Familiarity with the Net Health electronic documentation system is preferred.
• Strong clinical, communication, and interpersonal skills.
• Ability to effectively work with patients and residents with a variety of personalities, needs, and abilities.
• Strong organizational skills and attention to detail.
• Ability to collaborate effectively with patients, families, caregivers, and members of the interdisciplinary team.
• Commitment to providing compassionate, patient-centered care.

Physical Therapist Requirements

• Active and valid Physical Therapy license in PA.
• Must be in good standing with all applicable regulatory agencies and licensing boards.
