Job summary

Transitions At Home, Inc. is seeking a Part-Time Physical Therapist to join our home health team. You will deliver skilled therapy services at patients’ homes, following physician orders and individualized care plans to promote recovery and independence.

This role offers flexible daytime hours, no holidays or weekends, and a supportive interdisciplinary team to ensure high-quality patient care and meaningful patient relationships.

Qualifications

• Active Wisconsin Physical Therapist license.
• CPR certification.
• Valid WI driver’s license and car insurance.
• Comfortable working independently in a home care environment.

Responsibilities

• Develop and revise care plans.
• Perform skilled physical therapy procedures.
• Educate patients and families on home programs and daily activities.
• Collaborate with physicians and the care team.
• Participate in IDT meetings, in-service training, and QA initiatives.

Transitions At Home, is currently seeking to hire a Part-Time, compassionate and experienced Physical Therapist to join our growing home health team. Your Role:

As a Home Health Physical Therapist, you'll deliver skilled therapy services based on individualized care plans and physician orders. You'll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.

Key Responsibilities:
