Transitions At Home is seeking a compassionate Part-Time Physical Therapist to join our growing home health team in Wisconsin. The role offers independent scheduling with strong team support and a competitive hourly wage.

You will deliver skilled therapy services based on physician orders, collaborate with an interdisciplinary team, and help residents regain mobility and independence at home. Requires active WI PT license and CPR.

Qualifications

• Active Wisconsin Physical Therapist license
• CPR certification
• Valid WI driver’s license & car insurance
• Comfortable working independently in a home care environment

Your Role:

As a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.

Key Responsibilities:

• Develop and revise care plans
