Job summary

Transitions At Home for Central Wisconsin is seeking a Part-Time Licensed Physical Therapist Assistant to support residents in the Superior area. The PTA provides intermittent home-based physical therapy under physician orders and the supervision of the PT/RN Clinical Manager, helping patients regain independence and comfort.

Qualifications

• Active Wisconsin Physical Therapist Assistant license.
• Two years of experience preferred.
• CPR certification.
• A valid Wisconsin driver’s license and auto insurance.

Responsibilities

• Administers physical therapy in patients' homes under supervision.
• Follows physician orders and care plans.
• Assists in coordinating care with the Physical Therapist/RN Clinical Manager.
• Supports patient independence and comfort during treatment.

What does a Transitions at Home Physical Therapist Assistant do?
A Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Physical Therapist/RN Clinical Manager.
Hours: Part-Time

• 8:00am - 4:30pm
• Monday through Friday
• No Holidays

Qualifications / Job Requirements:

• Active Wisconsin Physical Therapist Assistant License
• Two (2) years’ experience preferred.
