Zusammenfassung

PRN Home Health & Therapy is seeking a licensed Physical Therapist to join our Wisconsin team. In this 0.4-1.0 FTE role, you'll deliver in-home evaluations, design individualized treatment plans, and promote mobility and independence for patients in their homes.

You'll collaborate with patients, families, and care teams, document progress, supervise PTAs, and participate in ongoing professional development while enjoying flexible scheduling and comprehensive benefits.

Qualifikationen

• Current Wisconsin Physical Therapist license.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but not required.
• Strong communication, compassion, and ability to work independently.
