Transitions At Home, Inc. is seeking a full-time Physical Therapist Assistant to deliver in-home physical therapy, helping patients regain independence with a compassionate, patient-focused approach.

The PTA will work a Monday–Friday schedule, 8:00am–4:30pm, in Southern Wisconsin, with a company that offers growth opportunities, a 401(k) plan with match, flexible spending accounts, tuition reimbursement, and other benefits.

Qualificações

• Active Wisconsin PTA license required.
• Two years of PTA experience preferred.
• CPR certification required.
• A valid Wisconsin driver’s license and car insurance are required.

Responsabilidades

• Provide intermittent physical therapy to patients in their homes under physician orders and plan of care.
• Deliver high-quality, compassionate care with supervision as needed by the administrator/clinical manager.

What does a Transitions at Home Physical Therapist Assistant do?

An Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager.

Hours: Full-Time

• 8:00am - 4:30pm
• Monday through Friday
• No Holidays

Qualifications / Job Requirements:
