Job summary

Fairview Hospital / Cleveland Clinic is seeking a licensed Physical Therapist to provide one-on-one care for older adults in private residences and senior living communities. You will assess patients, design individualized plans, and deliver evidence-based treatments while promoting mobility and independence.

The role involves collaboration with interdisciplinary teams, EMR documentation, patient/caregiver education, and occasional telehealth.

Qualifications

• Must hold a valid Physical Therapist license or be eligible for licensure.
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR systems and general computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in residences and senior living communities.
• Perform evaluations and develop personalized care plans.
• Deliver evidence-based treatments and support patient wellness and independence.
• Collaborate with interdisciplinary teams and document in EMR systems.
• Educate patients and caregivers; incorporate telehealth when appropriate.
• Travel to patient locations as needed.

Responsibilities
