Job summary

MEP Health is seeking a Licensed Physical Therapist to provide personalized in-home and senior living care, with optional virtual options, to older adults.

You will perform evaluations, develop treatment plans, document progress in EMR, collaborate with interdisciplinary teams, and educate patients and caregivers to support aging in place and safe mobility.

Qualifications

• Valid PT license and degree from an accredited program.
• Proficiency in computer use and EMR documentation.
• Current CPR certification.
• Ability to perform physical tasks including lifting, repositioning, and assisting with transfers.
