Personal Touch Home Health Care Services Inc. seeks a Physical Therapist to provide one-on-one therapy to older adults in private homes and senior communities.
The role includes evaluations, plan development, and evidence-based treatments to improve mobility and independence. You will document progress in EMR, collaborate with an interdisciplinary team, and educate patients, caregivers, and staff to promote safety and aging in place.
Role: Physical Therapist
Responsibilities: Provide one-on-one physical therapy to older adults in various settings, including private homes and senior living communities. Conduct evaluations, develop individualized care plans, and deliver evidence-based treatments to enhance mobility, independence, and quality of life. Document patient progress in electronic medical records, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth and virtual care to extend access and flexibility.
Qualifications & Skills: Valid Physical Therapy license, degree from an accredited program, proficiency in EMR and basic computer skills, and current CPR certification. Ability to meet physical demands such as lifting, repositioning patients, and driving to patient locations. Capable of performing CPR and assisting with patient transfers.
