Job summary

Live Well Homecare in Oregon seeks a Licensed Physical Therapist to provide personalized care to older adults in private homes and senior living communities, delivering evidence-based therapy to improve mobility and independence.

You will conduct evaluations, develop treatment plans, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and support telehealth when appropriate.

Qualifications

  • Valid PT license and degree from an accredited program.
  • Proficiency with EMR systems and computers.
  • Current CPR certification.
  • Ability to meet physical demands (lifting, repositioning, transfers).
