Job summary

Embassy of Heartside is seeking a compassionate full-time Physical Therapist (PT) to join our in-house rehabilitation team in State College, PA. New graduates are welcome to apply.

You will evaluate residents, develop personalized treatment plans, and collaborate with an interdisciplinary team to improve mobility and quality of life. Join a supportive, in-house rehab program with flexible scheduling and competitive compensation.

Qualifications

• Active Physical Therapy license in PA, in good standing.
• Strong clinical, communication, and organizational skills.
• Knowledge of Net Health Electronic Documentation System preferred.
• Skilled nursing or long-term care experience preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
