Job summary

Cliveden Nursing & Rehab Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehab team. New graduates are encouraged to apply, with mentorship and support to grow your clinical skills and professional confidence.

You'll evaluate residents, develop individualized treatment plans, provide skilled PT services, educate patients and families, and document progress in an electronic system. Collaborative team environment and ongoing education are key.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania or eligibility to obtain.
• Strong communication and interpersonal skills.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and patients and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established plans of care.
• Educate patients, families, and caregivers regarding treatment goals and interventions.
• Monitor patient progress and modify treatment plans as appropriate.
• Complete timely and accurate documentation in accordance with company policies and state and federal regulations.
• Communicate patient status and progress with physicians, nursing staff, and interdisciplinary team members.
• Assist with scheduling and accurately complete daily treatment charges.
