Job summary

Azza HealthCare Agency seeks a Licensed Physical Therapist to deliver personalized in-home and senior living care to older adults, improving mobility, independence and quality of life. You will assess patients, create care plans, and provide evidence-based treatments in private homes and community settings.

Requirements include a valid PT license or eligibility, degree from an accredited program, CPR certification, and EMR proficiency.

Qualifications

Responsibilities

• Provide one-on-one therapy in private residences and senior communities.

• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to enhance access and flexibility.

Physical Requirements:

Ability to lift up to 50 lbs, assist with patient transfers, stand/walk for extended periods, climb stairs, and drive between locations. PPE usage as needed.
