Job summary

Apex Home Health, Hospice & Home Care is seeking a Licensed Physical Therapist to provide one-on-one, in-home therapy for older adults, supporting mobility, independence and quality of life in private homes and senior living communities.

You will perform evaluations, develop individualized treatment plans, document progress in EMR, and collaborate with an interdisciplinary team. Telehealth options extend access and flexibility for patients and caregivers.

Qualifications

• PT degree from an accredited program and licensure in the applicable state.
• Ability to perform evaluations and create treatment plans.
• Experience with EMR documentation and telehealth is a plus.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy.
• Support patient wellness, functional independence, and longevity through therapy sessions.
• Document patient progress accurately using electronic medical records.
• Collaborate with interdisciplinary teams including OT, SLP, and Fitness Specialists.

• Utilize telehealth and virtual care to extend access and flexibility.

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited Physical Therapy program

• Current CPR certification

Physical Requirements

Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive between locations, with physical demands including lifting, repositioning patients, and extended walking or standing.
