Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home care. You’ll evaluate clients, create personalized care plans, and deliver skilled PT interventions to improve strength, balance, mobility, and safety.

This role offers flexible scheduling as part-time or per diem, mileage reimbursement, mobile EMR, supportive leadership, and opportunities for career growth. Active MA PT license and DPT or MPT required; new grads welcome.

Qualifications

• Licensed MA Physical Therapist with active license.
• CPR certification required.
• 1+ year of PT experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile EMR.

Skills
