Job summary

BrightStar Care seeks a licensed Physical Therapist (PT) for part-time / per diem in the Concord, MA area (also serving Lexington & Woburn). You’ll assess home health patients, design personalized plans, and deliver in-home interventions to improve strength, balance, mobility and safety.

Ideal candidates have an Active MA PT license, a DPT or MPT, CPR certification, and at least 1 year of experience (new grads welcome).

Qualifications

• Active MA PT license is required.
• DPT or MPT from an accredited program required.
• CPR certification is required.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
