BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to perform in-home evaluations and design personalized care plans that restore strength and independence. The role supports clients in their familiar home environments, focusing on safety and mobility with a collaborative care team.

The position offers part-time or per diem scheduling, mileage reimbursement, and a patient-centered approach to care.

Qualifications

• Active Massachusetts PT license (MA)
• CPR certified
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle and valid driver’s license

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to improve strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
