Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to provide in-home care. This part-time/per diem role offers flexible scheduling and local visits, with pay ranging from $75 to $85 per hour plus mileage reimbursement.

The PT will assess, plan, and deliver interventions to restore strength, balance, mobility, and safety in clients’ home environments. Qualified candidates hold an MA PT license (DPT or MPT), CPR certification, and a valid driver’s license

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
