Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home therapy services on a part-time or per diem basis. You’ll assess clients, build personalized care plans, and help restore strength, balance, and independence in the comfort of their homes.

You’ll work with a multidisciplinary team, document progress with a mobile EMR, and receive mileage reimbursement. A compassionate approach and reliable transportation are important, with new grads welcome

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile EMR.
