Job summary

Trinity Healthcare Talent Services is seeking a Physical Therapist to join our skilled nursing facility team. This full-time position offers $41-$50 per hour based on experience, with 40 hours per week, Monday–Friday and occasional weekend rotation.

The role focuses on evaluations, personalized plans, mobility improvement, and collaboration with a multidisciplinary team, with emphasis on compassionate, patient-centered care and adherence to documentation standards.

Qualifications

• DPT/Bachelor's/Master's in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• 1–2 years of SNF or similar setting experience preferred.
• Strong communication and interpersonal skills; team player.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and develop individualized treatment plans.
• Implement and monitor treatment plans to improve mobility, strength, balance, and overall physical function.
• Utilize therapeutic exercises, manual therapy, and assistive devices to help patients achieve goals.
• Educate patients and families on proper techniques and exercises to continue at home.
• Collaborate with other healthcare professionals to ensure coordinated care for patients.
• Document all treatment and progress notes accurately and timely.
• Maintain a clean and organized treatment area.
• Stay up-to-date with current trends and developments in physical therapy.
