Job summary

Trinity Healthcare Talent Services is seeking a Physical Therapist for a 84-bed Skilled Nursing Facility in North Carolina. This full-time position offers an hourly wage of $41.53 to $46.35, with 40 hours per week Monday through Friday and no weekend rotation.

You will evaluate patients, develop individualized treatment plans, and collaborate with a multidisciplinary team to restore mobility, reduce pain, and promote independence.

Qualifications

• Must hold a PT degree from an accredited program.
• Current state PT license.
• 1–2 years of SNF or similar setting preferred.
• Strong knowledge of PT principles and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Basic computer skills and proficiency in electronic medical records.

Responsibilities

• Evaluate patients' physical abilities, limitations, and medical history to determine the most effective treatment plan.
• Develop and implement individualized treatment plans that include therapeutic exercises, manual therapy, and other modalities.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Educate patients and their families on exercises and activities to promote recovery and prevent future injuries.
• Collaborate with other healthcare professionals, such as doctors and nurses, to ensure comprehensive and coordinated care for patients.
• Maintain accurate and up-to-date patient records and documentation.
• Adhere to all legal and ethical standards and guidelines in the provision of physical therapy services.
