Job summary

The ideal candidate holds a DPT, current PT license or eligibility, CPR certification, and demonstrates proficiency with EMR systems and telehealth tools to support patient care across settings.

Qualifications

• Valid Physical Therapy license or eligibility in the practicing state(s).
• Degree from an accredited PT program.
• Proficiency in basic computer use and electronic medical records.
• Current CPR certification.

Responsibilities

• Conduct evaluations and create individualized treatment plans to improve mobility and independence.
• Document patient progress in electronic medical records per standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Provide care via telehealth and virtual tools to extend access.

Key Responsibilities

• Conduct evaluations, develop individualized treatment plans, and deliver evidence‑based therapy aimed at improving mobility, independence, and quality of life.
• Document patient progress accurately within electronic medical records, adhering to regulatory standards.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual tools to extend care access and flexibility.

Qualifications & Skills

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited PT program.

Physical Requirements

Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand/walk for extended periods, and drive between locations. PPE use is required when necessary.
