Job summary

Phoenix of Maple Heights is seeking a dedicated Physical Therapist (PT) to join our skilled nursing and long-term care rehabilitation team in Maple Heights, Ohio. This Full-Time role involves evaluating residents, developing individualized treatment plans, and delivering skilled PT services to improve strength, mobility and independence.

You will collaborate with an interdisciplinary team and document using Net Health and PCC to support efficient, resident-centered care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license or eligibility.
• Strong clinical, communication, and interpersonal skills.
• Team-oriented approach to resident care.
• SNF/LTC experience is a plus.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/LTC setting.
• Help residents improve strength, balance, mobility, transfers and functional independence.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, physicians, and the interdisciplinary care team.
• Participate in care planning and discharge planning.
• Complete accurate and timely documentation using Net Health and PCC.

What You'll Do

• Evaluate residents and develop individualized treatment plans
