Job summary

Phoenix of Maple Heights in Maple Heights, OH is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team and support our skilled nursing residents. As part of the rehab team, you’ll provide treatments, work with supervising therapists, document progress, and strive to improve residents’ mobility and independence in a compassionate, resident-centered environment.

Ohio PTA license and graduation from an accredited PTA program are required; SNF/LTC experience and

What You’ll Do

As a PRN PTA, you’ll work alongside our Physical Therapists and interdisciplinary team to help residents improve mobility, strength, function, and independence.

Responsibilities include:

• Provide physical therapy treatments according to the established plan of care
• Work closely with the supervising Physical Therapist
• Help residents achieve their functional and mobility goals
• Accurately document treatments and resident progress
• Communicate with the rehab and interdisciplinary teams

What We’re Looking For

• Current Ohio Physical Therapist Assistant license
• Graduate of an accredited PTA program
• SNF/LTC experience is a plus
• Experience with Net Health is a plus
• Dependable, positive, and team-oriented
• Strong communication and documentation skills
