Job summary

Phoenix of Maple Heights is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Maple Heights, OH. You will support mobility, strength, and independence for residents in a SNF/LTC setting.

As part of the rehab team you’ll document progress, collaborate with therapists, and contribute to resident-centered care with a flexible PRN schedule.

Qualifications

• Current Ohio Physical Therapist Assistant license.
• Graduate of an accredited PTA program.
• SNF/LTC experience is a plus.
• Experience with Net Health is a plus.
• Dependable, positive, and team-oriented.
• Strong communication and documentation skills.

Responsibilities

• Provide physical therapy treatments according to the established plan of care.
• Work closely with the supervising Physical Therapist.
• Help residents achieve their functional and mobility goals.
• Accurately document treatments and resident progress.
• Communicate with the rehab and interdisciplinary teams.
• Provide compassionate, resident-centered care.

What You’ll Do
