Job summary

Phoenix of Maple Heights is seeking a Physical Therapist to join our full-time rehabilitation team in Maple Heights, OH. You will evaluate residents, develop treatment plans and provide skilled PT services in a SNF/LTC setting.

The ideal candidate holds an accredited PT degree and Ohio license (or eligibility), with strong clinical and communication skills and a team-oriented approach to resident care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license or eligibility to obtain.
• Strong clinical, communication and interpersonal skills.
• Team-oriented approach to resident care.
• SNF/LTC experience is a plus.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/LTC setting.
• Help residents improve strength, balance, mobility, transfers and functional independence.
• Monitor progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, physicians and the care team.
• Participate in care planning and discharge planning.
• Complete accurate documentation using Net Health and PCC.
• Provide compassionate, resident-centered care.

to join our dedicated rehabilitation team in Maple Heights, Ohio! What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in a SNF/LTC setting
• Help residents improve strength, balance, mobility, transfers, and functional independence

• Collaborate with nursing, therapy, physicians, and the interdisciplinary care team
• Participate in care planning and discharge planning
• Complete accurate and timely documentation using Net Health and PointClickCare (PCC)
• Provide compassionate, resident-centered care

What We're Looking For
