Qualifications

• Monitor progress and adjust plans to maximize functional outcomes.
• Supervise and collaborate with PTAs to ensure high-quality care.
• Recommend assistive devices and environmental modifications for safety.
• Educate residents and families on exercises and discharge planning.
• Maintain accurate clinical documentation.
• Participate in interdisciplinary team meetings and care planning.
• Adhere to regulatory standards and facility policies.
