Job summary

Phoenix Health Services LLC in Maple Heights, OH is seeking a PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. This position offers flexible scheduling and the chance to gain experience with residents in a long-term care setting.

As a PTA, you will work under the supervision of a licensed Physical Therapist to provide skilled therapy services, document progress, and collaborate with nurses and the interdisciplinary care team to improve residents' mobility,

Qualifications

• Active Ohio PTA license (or in good standing) and licensure compliance.
• Strong communication, organizational, and teamwork skills.
• Experience with geriatric populations preferred but not required.

Responsibilities

• Provide skilled PTA services under the supervision of a licensed Physical Therapist.
• Implement individualized treatment plans and address mobility, balance, endurance, and functional goals.

Phoenix Health Services at Phoenix of Maple Heights is looking for a PRN Physical Therapy Assistant (PTA) to join our in-house rehabilitation team in Maple Heights, Ohio.

What You'll Do

As a PTA, you'll work under the supervision of a licensed Physical Therapist to provide skilled, hands-on therapy services that help residents improve their strength, mobility, balance, independence, and quality of life.

• Providing skilled therapy interventions under the direction of a Physical Therapist
• Implementing individualized treatment plans
• Addressing strength, mobility, balance, endurance, and functional needs
• Monitoring resident progress and response to treatment
• Educating residents, families, and caregivers on therapy goals and techniques
• Communicating resident progress and concerns to the supervising PT
• Completing timely and accurate documentation
• Collaborating with nursing and the interdisciplinary care teamProviding compassionate, resident-centered care

What We're Looking For

• Active Ohio Physical Therapist Assistant (PTA) license
• License in good standing with applicable licensing boards and regulatory agencies
• LTC/SNF or geriatric experience preferred, but not required
• Net Health experience is a plus
• Strong communication, organizational, and teamwork skills
• Ability to work effectively as part of a collaborative therapy team
• Passion for helping residents maintain mobility, function, and independence
