Job summary

Phoenix Health Services at Phoenix of Maple Heights is seeking a PRN Physical Therapy Assistant (PTA) to join our in-house rehabilitation team in Maple Heights, Ohio. You will provide skilled therapy under the supervision of a licensed Physical Therapist to help residents improve strength, mobility, balance, independence, and quality of life.

This flexible PRN opportunity offers work when you are available with no set schedule and potential to gain additional hours.

Qualifications

• Active Ohio PTA license.
• License in good standing with licensing boards.
• LTC/SNF or geriatric experience preferred.
• Net Health experience is a plus.
• Strong communication, organizational, and teamwork skills.

Responsibilities

• Provide skilled therapy interventions under the direction of a Physical Therapist.
• Implement individualized treatment plans.
• Address strength, mobility, balance, endurance, and functional needs.
• Monitor resident progress and response to treatment.
• Educate residents, families, and caregivers on therapy goals and techniques.
• Communicate resident progress to the supervising PT.
• Complete timely and accurate documentation.
• Collaborate with nursing and the interdisciplinary care team.

What You\'ll Do

As a PTA, you\'ll work under the supervision of a licensed Physical Therapist to provide skilled, hands‑on therapy services that help residents improve their strength, mobility, balance, independence, and quality of life.

Responsibilities include:

What We\'re Looking For

• License in good standing with applicable licensing boards and regulatory agencies


• LTC/SNF or geriatric experience preferred, but not required


• Net Health experience is a plus
