Job summary

Qualifications

Responsibilities

• Provide PT services to outpatient and inpatient populations.
• Support rehab across settings (home health, SNF) as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain accurate clinical documentation using EMR systems.

Job Overview

We are seeking a dedicated Physical Therapist (PT) to join a collaborative healthcare team in International Falls, Minnesota. This role primarily focuses on outpatient care, with some inpatient responsibilities and occasional opportunities to support skilled nursing facilities (SNF), home health, and local school-based services.

The team is highly engaged and committed to providing quality, community-focused care across multiple patient settings.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services to outpatient and inpatient populations.
• Support rehabilitation needs across additional settings, including home health and skilled nursing facilities, as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain accurate clinical documentation using electronic medical records.
• Collaborate with an experienced, multidisciplinary healthcare team.

Requirements

• Active Minnesota Physical Therapist (PT) license.
• At least one year of experience in a health-related field.
• Technical proficiency with electronic medical records; Epic experience preferred.
• Basic Life Support (BLS) certification required within one year of hire.
• Strong communication, clinical, and patient-care skills.
• Monday–Friday day shifts.
• Weekend rotation approximately every 6–7 weeks.
• Holiday rotation is rare.
