The role involves initial evaluations, development of treatment plans, therapeutic exercises, and collaboration with care teams while adhering to home health and Medicare guidelines. CPR and a valid driver's license are required.

Qualifications

• Current Physical Therapist license required.
• Graduate of accredited PT program.
• 1–2 years of Home Health experience preferred.
• Strong communication skills.
• Ability to work independently.
• CPR certification.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Our company is expanding and we are seeking a skilled Home Health Physical Therapist (PT) to join our team.
