Job summary

Diamond Care Group is expanding and seeks a skilled Home Health Physical Therapist to provide individualized therapy in patients’ homes in Fort Myers, FL. You will help patients improve mobility and regain independence after illness, injury, or surgery.

You will perform initial evaluations, develop treatment plans, and educate patients and caregivers while adhering to Home Health and Medicare guidelines. Valid FL PT license and CPR are required, with reliable transportation.

Qualifications

• Must hold a valid Physical Therapist license in Florida.
• Graduate of an accredited PT program.
• 1–2 years of experience in Home Health preferred.
• CPR certification required.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS preferred.
• Experience with EMR systems such as Kinnser.

Responsibilities
