Job summary

AdviniaCare is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to help residents reach their highest functional level, focusing on independence, safety, and quality of life.

As a licensed PT, you will work with residents and families, coordinate with the rehabilitation team, and document progress and outcomes to ensure high-quality care.

Qualifications

• Licensed Physical Therapist credential required.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.
• Educate residents and their families on therapeutic exercises and techniques.
• Monitor and report on resident progress and outcomes.
• Maintain accurate and up-to-date records of patient care.
