Advinia Care is seeking a compassionate Physical Therapist to join our interdisciplinary team in Massachusetts. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

This role emphasizes collaboration with the rehab team and education for residents and families. As a licensed PT in MA, you will contribute to high-quality patient care, monitor progress, and maintain thorough documentation.

• Must be a licensed Physical Therapist in MA.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.
• Educate residents and their families on therapeutic exercises and techniques.
• Monitor and report on resident progress and outcomes.
• Maintain accurate and up-to-date records of patient care.
