Job summary

AdviniaCare is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

As a licensed Physical Therapist, you will work closely with the team to assess needs, educate residents and families, and monitor progress while maintaining accurate records. Growth opportunities exist within the facility.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals.
• Implement and modify treatment plans to address physical, cognitive, and emotional needs.
• Collaborate with interdisciplinary team to ensure coordination of care.

• Monitor and report on resident progress and outcomes.
• Maintain accurate and up-to-date records of patient care.
