As a licensed PT, you will collaborate with rehabilitation staff, educate residents and families, and monitor progress, ensuring thorough documentation of care.

Qualifications

• Licensed Physical Therapist must be licensed in the state of Rhode Island.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
