Job summary

AdviniaCare in Rhode Island is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for our residents.

The role requires a Licensed Physical Therapist and offers a $5,000 sign-on bonus, along with CEU/licensure reimbursement after a 90-day probation. You will collaborate with the care team to monitor progress and educate residents and families.

Qualifications

• Must be licensed to practice physical therapy.
• Experience with rehabilitation teams preferred.
• Strong communication with residents and families.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
