Job summary

AdviniaCare Orchard View in Riverside, RI is seeking a compassionate Physical Therapist to join our interdisciplinary rehabilitation team. You will design and implement personalized treatment plans to promote independence, safety, and quality of life for residents.

The role collaborates with clinicians, educates residents and families on exercises, and monitors progress while maintaining accurate records. A valid Rhode Island PT license is required.

Qualifications

• Licensed Physical Therapist in Rhode Island.
• Experience with rehabilitation teams is preferred.
• Ability to develop individualized treatment plans.

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
