Job summary

Embassy of Woodland Park in Orbisonia, PA seeks a compassionate Physical Therapist (PT) for part-time or PRN coverage. New graduates are welcome to apply, with an in-house rehabilitation program to support resident mobility, strength, and independence.

You will develop individualized treatment plans, provide skilled therapy, and collaborate with an interdisciplinary care team to enhance quality of life for residents while maintaining compliant documentation and standards.

Qualifications

• Active Physical Therapy license in PA in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
