Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide high-quality, individualized home-based care within a flexible schedule and supportive team environment.

Field Physical Therapist will perform SOC evaluations, develop care plans, and collaborate with an interdisciplinary team to meet each patient's goals, while educating families on home exercise programs.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities
