Job summary

Aventura at Walton Hills in Walton Hills, OH is seeking a PRN Physical Therapist (PT) to join our in-house therapy team. Flexible schedule and competitive PRN rates are offered to support residents' mobility and independence in a long-term care setting.

You'll evaluate residents, develop individualized plans, monitor progress and collaborate with PTAs and the interdisciplinary team to ensure safe, effective rehabilitation. This role is on-site with flexible shifts.

Qualifications

• Active Ohio Physical Therapist (PT) license.
• Long-term care or geriatric rehabilitation experience preferred but not required.
• Net Health documentation experience is a plus.
• Strong clinical, communication, and decision-making skills.

Responsibilities

• Evaluating residents to identify mobility limitations, strength deficits, balance concerns, and functional impairments
• Developing and implementing individualized treatment plans focused on mobility, gait training, strength, and pain management
• Monitoring resident progress and adjusting treatment plans as needed
• Supervising and collaborating with Physical Therapist Assistants (PTAs)
• Recommending assistive devices and adaptive equipment to promote safety and independence
• Educating residents, families, and caregivers on treatment plans and mobility strategies
• Maintaining timely and accurate clinical documentation
• Participating in interdisciplinary team meetings and care planning
• Following applicable clinical standards, regulatory requirements, and facility policies

What You'll Do

What We're Looking For

• Active Ohio Physical Therapist (PT) license
• License in good standing with applicable licensing boards and regulatory agencies
• Long-term care, skilled nursing, or geriatric rehabilitation experience preferred, but not required
• Net Health documentation experience is a plus
• Strong clinical, communication, and decision-making skills
• Ability to collaborate effectively with residents, families, and the interdisciplinary care team
• Passion for helping residents improve or maintain their independence and quality of life
