Job summary

Aventura at Walton Hills in Walton Hills, OH is seeking a PRN Physical Therapist (PT) to join our in-house therapy team. This flexible PRN role offers opportunities for experienced clinicians or those seeking additional experience in long-term care.

You will contribute to residents’ mobility and independence within a collaborative setting. Responsibilities include evaluating residents, creating individualized plans, and coordinating with an interdisciplinary team to ensure high-quality care.

Qualifications

• Active Ohio PT license in good standing.
• Strong clinical, communication, and decision‑making skills.
• Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve mobility, strength, balance, safety, and functional independence.
• Implement and adjust treatment plans focusing on mobility, gait training, strength, and pain management.
• Monitor progress and modify plans as needed.
• Supervise and collaborate with PTAs.
• Recommend assistive devices to promote safety and independence.
• Educate residents, families, and caregivers on plans and mobility strategies.
• Maintain timely clinical documentation and participate in interdisciplinary care planning.
