Job summary

Aventura at Carriage Inn in Dayton, OH is seeking a motivated Physical Therapist to join our in-house rehabilitation team. We are hiring for a Full-Time PT with PRN opportunities, welcoming new graduates as well as experienced therapists.

As part of an interdisciplinary team, you’ll evaluate residents, develop individualized PT plans, and help residents improve strength, mobility, and independence in a SNF/LTC setting. Ohio licensure is required; join us to make a meaningful impact.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Ohio Physical Therapist license.
• SNF/LTC experience is a plus, but not required.
• New graduates are encouraged and welcome to apply!

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide skilled PT services to our SNF/LTC population
• Help residents improve strength, mobility, balance, endurance, and overall functional independence
• Establish measurable goals and monitor resident progress
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Utilize Net Health and PointClickCare (PCC)
• Participate in discharge planning and resident/family education
• Maintain compliance with facility, state, and federal guidelines

We are seeking a motivated and compassionate Physical Therapist (PT) to join our team in Dayton, Ohio. Our primary need is for a Full-Time PT, with PRN opportunities also available.
