Job summary

Aventura Healthcare Group in Walton Hills, OH is seeking a Physical Therapist Assistant (PTA) for both full-time and PRN opportunities to join our in-house rehabilitation team.

You will work under the supervision of a licensed Physical Therapist to deliver skilled therapy, improve residents' mobility and independence, and participate in interdisciplinary care. We value compassionate, resident-centered care and flexible scheduling for FT and PRN hours.

Qualifications

• Active Ohio Physical Therapist Assistant (PTA) license.
• License in good standing with applicable licensing boards.
• LTC/SNF, geriatric, or post-acute experience preferred, but not required.
• Net Health experience is a plus.
• Strong communication, organization, and teamwork skills.
• Ability to work effectively as part of a collaborative therapy team.
• Passion for helping residents maintain mobility, function, and independence.

Responsibilities

• Providing skilled therapy interventions under the direction of a Physical Therapist
• Implementing individualized treatment plans
• Addressing strength, mobility, balance, endurance, and functional needs; Monitoring resident progress and response to treatment
• Educating residents, families, and caregivers on exercises and mobility strategies
• Communicating resident progress and concerns to the supervising PT
• Completing timely and accurate documentation
• Collaborating with nursing and the interdisciplinary care team
• Providing compassionate, resident-centered care

What You'll Do
