Job summary

Aventura at Walton Hills is seeking a Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Walton Hills, OH. We offer Full-Time and PRN opportunities with benefits for eligible employees.

As a PTA, you will work under a licensed Physical Therapist to deliver skilled therapy, support mobility, balance, and independence for residents, with flexible scheduling and a collaborative work environment.

Qualifications

• Active Ohio PTA license and in good standing.
• LTC/SNF or geriatric/post-acute experience preferred.
• Strong communication and teamwork skills.
• Net Health experience is a plus.

Responsibilities

• Provide skilled therapy interventions under the direction of a Physical Therapist.
• Implement individualized treatment plans.
• Address strength, mobility, balance, endurance, and functional needs.
• Monitoring resident progress and response to treatment.
• Educating residents, families, and caregivers on exercises and mobility strategies.
• Communicating resident progress and concerns to the supervising PT.
• Completing timely and accurate documentation.
• Collaborating with nursing and the interdisciplinary care team.
• Providing compassionate, resident-centered care.

What You'll Do

As a PTA, you'll work under the supervision of a licensed Physical Therapist to provide skilled, hands‑on therapy services that help residents improve their strength, mobility, balance, endurance, independence, and overall quality of life.

What We’re Looking For

• License in good standing with applicable licensing boards and regulatory agencies
• LTC/SNF, geriatric, or post-acute experience preferred, but not required
• Net Health experience is a plus
• Strong communication, organization, and teamwork skills
• Ability to work effectively as part of a collaborative therapy team
• Passion for helping residents maintain mobility, function, and independence
