Job summary

Tljohnie is seeking a Physical Therapist to provide home health care services, assess functional levels, and develop treatment plans to improve independence in patients’ homes.

You will educate patients and caregivers, supervise support staff, and report progress regularly. Requires IL licensure and a valid driver’s license with local travel to residences.

Qualifications

• Active Physical Therapist license in Illinois (IL).
• Valid Regular Driver's License (DL).

Responsibilities

• Perform Physical Therapy assessments and develop individualized treatment programs.
• Record and report patient's reaction to treatment and changes in condition.
• Instruct patients and caregivers in therapy programs and proper device use.
• Supervise physical therapy assistants and, when appropriate, certified nursing assistants.
• Prepare reports and participate in staff and Agency development activities.

Position Summary

Qualifications Required

• Education: Master's degree or higher
• Experience: Healthcare Setting (1+ years)
• Licenses/Certifications: Valid Physical Therapist license in the state of Illinois (IL) Valid Regular Driver's License (DL)
• Preferred: PT license number (to be provided during application)

Overview

Schedule

Monday – Friday, 8: 00a – 5:00p Plus one weekend per month and 2 holidays per year Typically sees 5–6 patients per day

Travel: Local travel to residential homes required
