Job summary

Paycom - ATS is seeking a Physical Therapist to provide safe, effective care through assessment, planning and evaluation under the Illinois Physical Therapy Practice Act. You will promote optimal recovery and coordinate with physicians and the care team for adults and geriatrics.

The role requires an Illinois license, a PT degree, and at least one year of health care experience with home health services. Computer skills and reliable transportation are essential.

Qualifications

• Graduate of a physical therapy curriculum approved by the American Physical Therapy Association.
• Licensed in the State of Illinois.
• One year of experience in a health care setting is required to provide home health services.
• Must be able to complete computer work.

Job Summary: The Physical Therapist is responsible for providing safe and effective delivery of patient care through assessment, planning and evaluation of patient and family needs within the parameters of the Illinois Physical Therapy Practice Act in order to deliver care promoting optimal recovery and rehabilitation. The Physical Therapist promotes a safe and effective care to adults to coordinate care with the physician and any others involved in the patients care. Age specific competency is measured by a variety of methods which could include, but is not limited to: self-learning packets, review of age-specific guidelines, direct observation, video presentations, department- specific class, policies and procedures, chart review and goals.Job Responsibilities:Supports patient’s rights and responsibilities, including respect of cultural and religious diversity.Demonstrates flexibility to meet staffing needs of department/organization and appearance.Meets productivity standards, accepting low census time and/or extra time as needed.Demonstrates leadership abilities while appropriately following the chain of command.Performs and evaluates appropriate and complete evaluation based on patient diagnoses and specific to care, leading to appropriate care planning decisions.Provides age-appropriate and population specific to care in a safe, skillful, accurate and efficient manner to adults and geriatrics.Assesses patient and family learning needs and provides ongoing education and evaluation.
