Chestnut Health is seeking a Physician Assistant to join our physiatry team in a patient-centered rehabilitation setting for SNF patients. The role focuses on collaborative care with therapists, nurses, and other clinicians to improve function and independence.

Qualifications

• Active licensed to practice in the state of employment.
• Current DEA license required upon start date.
• NCCPA certification is required.
• Desire to work in Physiatry/PM&R.

Responsabilités

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for rehabilitation patients.
• Conduct thorough patient assessments including histories and examinations to set rehabilitation goals.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities to enhance clinical skills.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
• Provide education and counseling to patients and families about rehabilitation and post-discharge care.

Certified Physician Assistant (PA) Certification

Active State PA License (Arizona)

Current DEA License

Current Controlled Substance License

NCCPA Certificate
